On March 20, 2025, the NRC issued a notice of violation (Notice) and proposed imposition of civil penalty in the amount of $14,400 to Saint Francis Medical Center (licensee) for a Severity Level II problem associated with four related violations. The violations involved the licensee’s failure to: (1) have a written directive that was dated and signed by an authorized user before the administration of a therapeutic dosage of unsealed byproduct material, as required by Title 10 of the Code of Federal Regulations (10 CFR) 35.40(a); (2) provide high confidence that: (1) the patient’s identity was verified before each administration; and (2) each administration was in accordance with the written directive, as required by 10 CFR 35.41(a); (3) provide training to all individuals involved in yttrium-90 microsphere use, commensurate with the individual’s duties, as required by the licensee’s License Condition 14.C; and (4) ensure that radiation safety activities were being performed in accordance with licensee-approved procedures and regulatory requirements, as required by 10 CFR 35.24(b).
